Dosage and Administration

Always follow your doctor’s instructions for dosage. The following is a general guide.

Adult Dose

  • HIV Infection: For the first 14 days, take 200 mg once daily. If you don’t develop a rash, the dose is then increased to 200 mg twice daily.
  • If treatment is stopped for more than seven days, you’ll need to restart at the initial lower dose for 14 days.

Pediatric Dose

  • HIV Infection (2 months to 16 years): Dosage is based on weight and age.
    • Ages 2 months to 8 years: 4 mg/kg once daily for 14 days, then increase to 7 mg/kg twice daily if no rash appears.
    • Ages 8 to 16 years: 4 mg/kg once daily for 14 days, then 4 mg/kg twice daily.
  • The maximum daily dose is 400 mg.
  • As with adults, if treatment is interrupted for more than seven days, restart with the initial lower dose for 14 days.

Other Instructions

  • Administration: You can take Nevir with or without food.
  • Renal Impairment: If you are on hemodialysis, an additional 200 mg dose is recommended after each dialysis session.

Important Precautions and Warnings

  • Contraindications: Do not use Nevir if you have a known hypersensitivity to nevirapine, severe liver impairment, or are breastfeeding.
  • Liver and Skin Reactions: Nevirapine can cause severe, life-threatening skin rashes and liver damage. Your doctor will monitor your liver function regularly. Stop taking the medication immediately and contact your doctor if you develop a severe rash or signs of liver problems.
  • Pregnancy: If you are pregnant or may become pregnant, discuss the potential risks with your doctor.
  • Drug Interactions: Nevirapine can affect the levels of other drugs, particularly those metabolized by the CYP3A enzyme, such as methadone, which may have reduced effects.

Mechanism of Action

Nevirapine is a type of medication called a non-nucleoside reverse transcriptase inhibitor (NNRTI). It works by directly binding to and disrupting the reverse transcriptase enzyme, which HIV-1 needs to replicate. This action prevents the virus from making copies of its genetic material.

Possible Side Effects

Common side effects of Nevirapine include:

  • Skin rash
  • Nausea, vomiting, and abdominal pain
  • Headache
  • Fatigue
  • Diarrhea
  • Abnormal liver function tests (LFTs)
