Zapacid Capsule, with the generic name Lansoprazole, is a proton pump inhibitor (PPI) manufactured by Win-Medicare Pvt. Ltd. It’s available in Nepal.

Uses (Indications)

Zapacid Capsule is used to treat conditions related to excess stomach acid, including:

  • Peptic ulcers (duodenal and gastric ulcers)
  • H. pylori infection (in combination therapy)
  • Gastro-oesophageal reflux disease (GERD)
  • Hypersecretory conditions (e.g., Zollinger-Ellison syndrome)
  • Acid-related dyspepsia (indigestion)
  • NSAID-induced ulcers (for treatment and prevention)
  • Erosive oesophagitis (inflammation and erosion of the esophagus)

How it Works (Mechanism of Action)

Lansoprazole is a substituted benzimidazole, also known as a PPI. It works by blocking the final step of acid secretion by inhibiting the H+/K+ ATPase enzyme system in the gastric parietal cells of your stomach. This inhibits both basal (constant) and stimulated acid production.

Dosage Information

Adults:

  • Peptic Ulcer:
    • Duodenal Ulcer: 30 mg once daily in the morning for up to 4 weeks.
    • Gastric Ulcer: 30 mg once daily in the morning for up to 8 weeks.
    • Maintenance: 15 mg once daily.
  • Zollinger-Ellison Syndrome: Initially, 60 mg once daily in the morning; adjust as needed. For daily doses over 120 mg, take in 2 divided doses.
  • Acid-Related Dyspepsia: 15-30 mg once daily in the morning for 2-4 weeks.
  • Gastro-oesophageal Reflux Disease (GERD): 15-30 mg once daily in the morning for 4-8 weeks.
    • Maintenance: 15-30 mg once daily, adjusted based on response.
  • Prophylaxis of NSAID-induced Ulcers: 15-30 mg once daily in the morning.
  • H. pylori Infection (1-week triple therapy): 30 mg twice daily combined with:
    • Clarithromycin 500 mg twice daily and amoxicillin 1 g twice daily, OR
    • Clarithromycin 250 mg twice daily and metronidazole 400 mg twice daily.
  • Erosive Oesophagitis: 30 mg once daily in the morning for up to 8 weeks; an additional 8 weeks may be needed if not healed.
    • Maintenance: 15 mg once daily.
  • NSAID-Associated Ulceration: 30 mg once daily in the morning for 4-8 weeks.
  • Elderly: No dosage adjustment is typically needed.
  • Severe Hepatic Impairment: Maximum dose is 30 mg per day.

Children (Oral):

  • Gastro-oesophageal Reflux Disease (GERD) & Erosive Oesophagitis:
    • 1-11 years:
      • Weight <30 kg: 15 mg once daily in the morning for up to 12 weeks.
      • Weight >30 kg: 30 mg once daily in the morning for up to 12 weeks.
      • If symptoms persist after 2 or more weeks, doses may be increased up to 30 mg twice daily.
    • 12-17 years: 15 mg once daily for up to 8 weeks for GERD; 30 mg once daily for up to 8 weeks for erosive oesophagitis.

Renal Impairment: No dosage adjustment is needed.

Administration

Take Zapacid Capsule on an empty stomach, before meals.

Important Considerations

Do Not Use If You Have (Contraindications):

  • Hypersensitivity (allergy) to Lansoprazole or any of its components.

Use with Caution If You Have (Precautions):

  • Hepatic impairment (liver problems).
  • Your doctor should rule out gastric malignancy (stomach cancer) before starting treatment.
  • Pregnancy and lactation: Use only if clearly needed.
    • Lactation: It’s unknown if Lansoprazole passes into breast milk; breastfeeding is generally not recommended while on this medication.

Potential Drug Interactions

  • Increased Risk:
    • Hypomagnesaemia (low magnesium levels) when used with diuretics and digoxin.
    • Plasma concentrations of cilostazol and methotrexate.
  • Decreased Plasma Concentration/Effect:
    • Erlotinib, dasatinib, and lapatinib.
    • Bioavailability of itraconazole and ketoconazole.
    • Reduced bioavailability when taken with antacids and sucralfate.
  • Potentially Fatal: May significantly decrease serum levels and pharmacological effects of rilpivirine and atazanavir.

Possible Side Effects (Adverse Effects)

Common (1-10%):

  • Headache (3-7%)
  • Diarrhea (1-5%)
  • Constipation (1-5%)
  • Nausea (1-3%)
  • Abdominal pain (1-3%)

Less Common (<1%):

  • Anxiety, angina, palpitations, syncope, edema, anorexia, dry mouth, tenesmus, flatulence, melena, myalgia, tinnitus, allergic reaction.
