Description

Sobica Tablet (Sodium Bicarbonate)

Indications:

  • Dyspepsia (indigestion)
  • Urine alkalinization (to prevent uric acid renal calculi, especially during uricosuric therapy for hyperuricemia in chronic gout)
  • Severe metabolic acidosis
  • Hyperkalemia (high potassium levels)

Dosage and Administration:

Adults:

  • Oral:
    • Urine Alkalinization: Up to 10 grams daily in divided doses, taken with a generous amount of fluid, particularly in the initial stages of uricosuric therapy for hyperuricemia in chronic gout (to prevent uric acid renal calculi).
    • Chronic Metabolic Acidosis: Doses providing 57 mmol (4.8 grams of sodium bicarbonate) or more daily, as required, based on patient needs and acid-base status.
    • Dyspepsia: 1-5 grams dissolved in water, may be taken as needed for symptomatic relief.
  • Intravenous (for Severe Metabolic Acidosis):
    • Administered by slow injection of a hypertonic solution (up to 8.4% or 1000 mmol/L) or by continuous infusion of a weaker solution (typically 1.26% or 150 mmol/L).
    • For correction of acidosis during advanced cardiac life support (ACLS) procedures, 50 mL of an 8.4% solution may be given.

Special Populations:

  • Elderly: Dosage adjustments may be necessary.
  • Hepatic Impairment: Dosage adjustments may be necessary.
  • Renal Impairment: Dosage adjustments may be necessary.

Administration Instructions:

  • Sobica Tablet should be taken on an empty stomach, at least one hour before food or two hours after food.

Contraindications:

  • Metabolic alkalosis
  • Respiratory alkalosis
  • Hypernatremia (high sodium levels)
  • Severe pulmonary edema
  • Hypocalcemia (low calcium levels)
  • Hypochlorhydria (low stomach acid)

Precautions:

  • Use with caution in patients with:
    • Epilepsy
    • Congestive heart failure (CHF)
    • Renal impairment
    • Liver cirrhosis
    • Hypertension (high blood pressure)
    • Edema (fluid retention)
    • Eclampsia
    • Aldosteronism
  • Monitoring: Serum electrolyte concentrations and acid-base status should be regularly monitored during treatment, especially for acidosis.
  • Pregnancy and Lactation: Use with caution.

Interactions:

  • Increased Toxicity: May increase the toxicity of amphetamines, ephedrine, pseudoephedrine, flecainide, quinidine, and quinine.
  • Decreased Effects: May decrease the effects of lithium, chlorpropamide, and salicylates due to increased renal clearance.
  • Altered Absorption: May affect the absorption of certain drugs due to increased intra-gastric pH.

Adverse Effects (Side Effects):

  • Aggravated congestive heart failure (CHF)
  • Cerebral hemorrhage
  • Edema (swelling)
  • Hypernatremia (high sodium levels)
  • Hypocalcemia (low calcium levels)
  • Hypokalemia (low potassium levels)
  • Tetany (muscle spasms)
  • Tissue necrosis at the injection site (with intravenous administration)

Mechanism of Action: Sodium bicarbonate exerts its therapeutic effects by dissociating to provide bicarbonate ions. These bicarbonate ions neutralize hydrogen ion concentration, thereby raising both blood and urinary pH. In the stomach, it neutralizes gastric acid through the production of carbon dioxide.
