Gastractiv 10mg Tablet is a medication manufactured by Johnson & Johnson Ltd. Its active ingredient is Domperidone (10mg). It comes in a pack size of 10 tablets and has a retail price of 4.10 NPR.

 

What is Gastractiv Used For?

 

Gastractiv Tablet is used to treat various conditions, including:

  • Nausea and Vomiting
  • Gastritis (inflammation of the stomach lining)
  • Gastroparesis (delayed stomach emptying)
  • Non-ulcer Dyspepsia (indigestion not caused by ulcers)
  • Migraine (to help with associated nausea and vomiting)
  • Lactation (to promote milk production in some cases)

 

How to Take Gastractiv

 

Always take Gastractiv on an empty stomach, ideally 15-30 minutes before meals.

Adult Dosing (Oral):

  • Nausea and Vomiting: 10-20 mg every 4-8 hours. Do not exceed 80 mg per day.
  • Non-ulcer Dyspepsia: 10 mg 3-4 times daily. The maximum dose is 20 mg 3-4 times daily, with the last dose taken at bedtime.
  • Migraine: 20 mg every 4 hours. Do not exceed 4 doses in 24 hours.

Adult Dosing (Rectal):

  • Nausea and Vomiting: 60 mg twice daily.

Dosing for Children (Oral):

  • 0.2 – 0.4 mg/kg every 4-8 hours daily.

Dosing for Children (Rectal):

  • Under 12 years and over 15 kg: 30 mg twice daily.
  • Over 12 years and over 35 kg: 60 mg twice daily.

Special Populations:

  • Elderly: No dosage adjustment is typically needed.
  • Hepatic Impairment (Liver Problems): No dosage adjustment is typically needed.
  • Renal Impairment (Kidney Problems):
    • Mild to Moderate: No dosage adjustment is typically needed.
    • Severe: Dosing may need to be reduced to 1-2 times daily with prolonged treatment.

 

Important Considerations

 

Do Not Take Gastractiv If You Have:

  • A known allergy to Domperidone or any other ingredients.
  • Gastrointestinal bleeding, obstruction, or perforation.
  • A prolactin-releasing pituitary hormone condition.
  • If you are planning chronic administration or routine prevention of post-operative nausea and vomiting.

Precautions:

Discuss with your doctor before taking Gastractiv if you have:

  • Pheochromocytoma (a rare tumor of the adrenal gland).
  • Are a child under 2 years old.
  • Are elderly.
  • Have renal (kidney) or hepatic (liver) impairment.
  • There’s a risk of cardiac arrhythmias and hypokalemia if administered intravenously (IV).
  • Are pregnant or breastfeeding.

Drug Interactions:

  • Gastractiv may reduce the effect of bromocriptine (a medication used for various conditions, including Parkinson’s disease).
  • Opioid analgesics (pain relievers) and antimuscarinics (medications that block certain nerve impulses) may reduce Gastractiv’s prokinetic (gut-motility enhancing) effect.
  • Potentially Fatal Interaction: Taking Gastractiv with strong CYP3A4 inhibitors (such as ketoconazole, erythromycin, or ritonavir) can significantly increase Domperidone levels in your blood, raising the risk of serious heart problems (QT prolongation).

 

Possible Side Effects

 

Common side effects of Domperidone can include:

  • Drowsiness
  • Extrapyramidal reactions (involuntary movements)
  • Galactorrhoea (milk production not associated with childbirth or breastfeeding)
  • Gynaecomastia (breast enlargement in males)
  • Constipation or diarrhea
  • Lassitude (lack of energy)
  • Decreased libido (sex drive)
  • Skin rash, itching

Potentially Serious Side Effects (Seek immediate medical attention if you experience these):

  • Convulsions (seizures)
  • Arrhythmias and cardiac arrest (irregular heartbeat and heart stopping)
  • Dysrhythmias (abnormal heart rhythm) in patients with pre-existing heart disease or low potassium levels (hypokalemia).
  • Seizures
  • Hypertensive crisis (severely high blood pressure) in patients with pheochromocytoma.

 

How Gastractiv Works

 

Domperidone, the active ingredient in Gastractiv, is a peripheral dopamine-receptor blocker. It works by:

  • Increasing the movement of the esophagus (food pipe).
  • Improving coordination between the stomach and small intestine.
  • Lowering pressure in the esophageal sphincter (the muscle that keeps food in the stomach).
  • Enhancing stomach motility and peristalsis (wave-like contractions), which helps food move through the digestive system faster.
  • Decreasing the time it takes for food to pass through the small bowel.
