Fixx Tablet (Cefixime)
Description: Fixx Tablet contains Cefixime, an antibiotic belonging to the cephalosporin class.
Indications: Fixx Tablet is prescribed for the treatment of various susceptible bacterial infections, including:
- Respiratory Tract Infections: Such as pneumonia, pharyngitis, sinusitis, tonsillitis, acute bronchitis, and acute exacerbations of chronic bronchitis.
- Ear Infections: Otitis media.
- Soft Tissue Infections.
- Typhoid Fever.
- Uncomplicated Gonococcal Urethritis.
Dosage and Administration:
Adults:
- For Acute Bronchitis & Acute Exacerbations of Chronic Bronchitis, Otitis Media, Pharyngitis/Tonsillitis, and Uncomplicated Urinary Tract Infections: The usual dose is 200 mg or 400 mg daily, taken either as a single dose or divided into two doses.
- For Gonorrhea (Uncomplicated): A single dose of 400 mg is recommended.
- For Typhoid Fever: The dose is 20 mg/kg body weight daily, divided into two doses. Treatment typically lasts 7 days but can be extended up to 14 days based on the severity of the infection.
Children:
- General Dosing (for children weighing less than 50 kg): The recommended dose is 8 mg/kg/day orally, administered every 12 to 24 hours.
- For Convalescent Oral Therapy of Serious Infections: The dose can be increased up to 20 mg/kg/day.
- For Powder for Suspension & DS Powder for Suspension (Specific Age-Based Dosing):
- Children above 6 months: 8 mg/kg daily in 1-2 divided doses.
- 6 months – 1 year: 75 mg daily.
- 1 – 4 years: 100 mg daily.
- 5 – 10 years: 200 mg daily.
- For Typhoid Fever: 15-20 mg/kg/day orally, divided every 12 hours for 7-14 days. The total daily dose should not exceed 400 mg.
- Children under 6 months: The safety and efficacy of Cefixime in this age group have not been established.
Renal Impairment:
- If you have impaired kidney function, a dose reduction is necessary.
- For a Creatinine Clearance (CrCl) of less than 20 mL/min, the maximum daily dose should not exceed 200 mg.
Administration: Fixx Tablet can be taken with or without food. Taking it with food or milk may help reduce any gastrointestinal discomfort.
Contraindications:
- Do not take Fixx Tablet if you have a known hypersensitivity to cephalosporins.
Precautions:
- Exercise caution if you have a history of allergy to penicillins.
- Use with care during pregnancy and lactation.
- Special consideration is needed for patients with renal failure or gastrointestinal (GI) disease.
Lactation: It is currently unknown whether Cefixime is excreted in breast milk.
Drug Interactions:
- Probenecid: Concomitant use with probenecid may lead to increased concentrations of Cefixime in the body.
- Anticoagulants: Cefixime may potentially increase prothrombin time when used with anticoagulants. Close monitoring is advised.
Adverse Effects:
- Common (>10%): Diarrhea (16%).
- Frequency Not Defined (may include): Abdominal pain, candidiasis, dizziness, dyspepsia, elevated transaminases, eosinophilia, erythema multiforme, fever, flatulence, headache, increased blood urea nitrogen (BUN), increased creatinine, leukopenia, nausea, prolonged prothrombin time (PT), pruritus, pseudomembranous colitis, rash, serum sickness-like reaction, Stevens-Johnson syndrome, thrombocytopenia, urticaria, vaginitis, and vomiting.
- Potentially Fatal: Pseudomembranous colitis.
Mechanism of Action: Cefixime works by binding to specific proteins within bacteria called penicillin-binding proteins (PBPs). This action inhibits the final step of peptidoglycan synthesis, which is crucial for building the bacterial cell wall. By preventing cell wall assembly, Cefixime effectively kills the bacteria.
