Dosing & Administration
- Adults: Typically 200-400 mg daily. For gonorrhea, it’s a single 400 mg dose. Typhoid fever is 20 mg/kg/day, usually for 7-14 days.
- Children: 8 mg/kg/day for most infections; 15-20 mg/kg/day for typhoid fever (not exceeding 400 mg/day). Not for infants under 6 months.
- Kidney Impairment: Dose reduction needed; maximum 200 mg daily if CrCl < 20 ml/min.
- Take with or without food; food or milk can lessen stomach upset.
Important Notes
- Do not use if allergic to cephalosporins.
- Use with caution if you have a history of penicillin allergy, kidney failure, or GI disease.
- Interactions: Can increase levels of probenecid and may dangerously increase bleeding risk with anticoagulants.
- Common Side Effect: Diarrhea (16%).

Indications: What Bestocef Syrup is Used For
Bestocef Syrup is prescribed for various susceptible infections, including:
- Respiratory Tract Infections: Pneumonia, Pharyngitis, Sinusitis, Otitis media, Tonsillitis, Acute Bronchitis, and Acute Exacerbations of Chronic Bronchitis.
- Typhoid Fever
- Soft Tissue Infections
- Gonococcal Urethritis
Dosing Information

Adult Dose
- Acute Bronchitis, Acute Exacerbations of Chronic Bronchitis, Otitis Media, Pharyngitis/Tonsillitis, Uncomplicated Urinary Tract Infections: 200 mg or 400 mg once daily, or in two divided doses.
- Gonorrhea: A single 400 mg dose.
- Typhoid Fever: 20 mg/kg body weight daily, divided into two doses. The usual treatment duration is 7 days, which may be extended up to 14 days based on the infection’s severity.
Child Dose
- For children weighing less than 50 kg: 8 mg/kg/day, given every 12 to 24 hours.
- For convalescent oral therapy of serious infections: Up to 20 mg/kg/day.
- Powder for Suspension & DS Powder for Suspension:
- Children above 6 months: 8 mg/kg daily in 1-2 divided doses.
- 6 months to 1 year: 75 mg daily.
- 1-4 years: 100 mg daily.
- 5-10 years: 200 mg daily.
- Typhoid Fever (children): 15-20 mg/kg/day orally, divided every 12 hours for 7-14 days. Do not exceed 400 mg/day.
- Note: Safety and efficacy have not been established for children under 6 months of age.
Renal Impairment (Kidney Issues)
Dose reduction is necessary for individuals with kidney impairment. If your Creatinine Clearance (CrCl) is less than 20 ml/min, the maximum daily dose should be 200 mg.
Administration
Bestocef Syrup can be taken with or without food. Taking it with food or milk may help reduce gastrointestinal discomfort.

Mechanism of Action
Cefixime works by binding to penicillin-binding proteins (PBPs) in bacteria. This action inhibits the final step of peptidoglycan synthesis, which is crucial for bacterial cell wall formation. By disrupting cell wall assembly, Cefixime leads to the death of the bacterial cells.
