Dosage Information

Adult Dosing

 

  • Oral (by mouth) for Moderate to Severe Pain:
    • Immediate Release:
      • Chronic Pain: Start with 25 mg every morning, increasing by 25-50 mg daily every 3 days. Doses can be taken as 50-100 mg every 4-6 hours as needed, but do not exceed 400 mg per day.
      • Acute Pain: Take 50-100 mg every 4-6 hours as needed, not to exceed 400 mg per day.
    • Extended Release: Start with 100 mg once daily, increasing by 100 mg daily every 5 days. Do not exceed 300 mg per day.
  • Parenteral (IM/IV Injection) for Moderate to Severe Pain:
    • Administer 50-100 mg every 4-6 hours over 2-3 minutes.
  • Parenteral (IM/IV Injection) for Postoperative Pain:
    • Initially, 100 mg, followed by 50 mg every 10-20 minutes if necessary, up to a total of 250 mg in the first hour (including the initial dose). Thereafter, 50-100 mg every 4-6 hours, up to a total daily dose of 600 mg.

 

Special Populations

 

  • Elderly (over 75 years): The dosing interval may need to be increased.
  • Hepatic (Liver) Impairment:
    • Severe Impairment: For immediate release, 50 mg every 12 hours. Extended release is not recommended.

 

Child Dosing

 

  • Moderate to Severe Pain (Immediate Release):
    • Under 17 years: Safety and efficacy have not been established.
    • Over 17 years (Acute): 50-100 mg every 4-6 hours as needed, not to exceed 400 mg per day.
    • Over 17 years (Chronic): Start with 25 mg every morning, increasing by 25-50 mg daily every 3 days in separate doses up to 50-100 mg every 4-6 hours as needed. Do not exceed 400 mg per day.
  • Extended Release:
    • Under 18 years: Safety and efficacy have not been established.

  • Increased risk of convulsions or serotonin syndrome: When taken with SSRIs, SNRIs, TCAs, and other seizure-threshold-lowering drugs (e.g., bupropion, mirtazapine, tetrahydrocannabinol).
  • Decreased serum concentrations of Anadol: When taken with carbamazepine.
  • May potentiate the antidepressant effect of norepinephrine, 5-HT agonists, or lithium.
  • Increased INR and ecchymoses: When taken with coumarin derivatives (e.g., warfarin).
