Altramox Drop (Amoxicillin)

 

Indications:

Altramox Drop is used to treat a wide range of bacterial infections, including:

  • Sore throat (Pharyngitis)
  • Ear infections (Acute otitis media)
  • Sinus infections (Acute bacterial sinusitis)
  • Heart valve infections (Endocarditis)
  • Anthrax
  • Certain sexually transmitted infections like Chlamydial cervicitis and Chlamydial urethritis
  • Lyme disease
  • Dental abscesses
  • Salmonella infections (Salmonellosis)
  • Typhoid fever
  • Uncomplicated gonorrhea
  • Infections affecting the lower respiratory tract, skin and skin structures, genitourinary tract, ear, nose, and throat.

Dosage and Administration:

Adults:

  • For most susceptible infections (by mouth): 250-500 mg every 8 hours.
  • For uncomplicated gonorrhea (by mouth, with probenecid): A single dose of 3 grams.
  • For dental abscesses (by mouth): 3 grams initially, then repeat once 8 hours later.
  • For uncomplicated acute urinary tract infections (UTI) (by mouth): 3 grams initially, then repeat once 10-12 hours later.
  • For endocarditis prevention (prophylaxis) before dental procedures (by mouth): 2 or 3 grams as a single dose, 1 hour before the procedure.
  • For severe or recurrent respiratory tract infections (by mouth): 3 grams twice daily.
  • For H. pylori infection (by mouth): 500 mg three times a day, typically in combination with metronidazole or clarithromycin, and a bismuth compound or an acid-reducing medication.
  • For susceptible infections (by injection – IV/IM): 500 mg every 8 hours.
  • For Listerial meningitis (by injection – IV, with other antibiotics): 2 grams every 4 hours for 10-14 days.

Children (by mouth):

  • Standard dose: 40–45 mg per kg of body weight per day, divided into doses given every 8-12 hours.
  • High dose: 80–90 mg per kg of body weight per day, with a maximum of 4 grams per day, divided into doses given every 12 hours.
  • For penicillin-resistant S. pneumoniae otitis media: 150 mg per kg of body weight per day, divided into doses given every 8 hours.

Renal Impairment (Kidney Problems):

  • Patients on hemodialysis: Should receive 250-500 mg every 24 hours, plus an additional dose during and after each dialysis session.
  • Creatinine Clearance (CrCl) 10-30 ml/min: 250-500 mg every 12 hours.
  • Creatinine Clearance (CrCl) <10 ml/min: 250-500 mg every 24 hours.

Administration:

Altramox can be taken with or without food. Taking it with meals may improve absorption and help reduce stomach discomfort.

Contraindications (When Not to Use):

  • Do not use if you have a hypersensitivity (allergic reaction) to amoxicillin or any other penicillin.

Precautions (Use with Caution):

  • If you have kidney or liver disease.
  • If you are pregnant or breastfeeding. Amoxicillin is excreted in breast milk, so caution is advised.
  • If you have infectious mononucleosis, as there’s an increased risk of rash.

Interactions with Other Medications:

  • May reduce the effectiveness of oral contraceptives (birth control pills).
  • May increase the effect of anticoagulants (blood thinners).
  • Increased risk of allergic reactions when taken with allopurinol.
  • Increased and prolonged blood levels of amoxicillin when taken with probenecid.
  • Chloramphenicol, macrolides, sulfonamides, and tetracyclines may interfere with the bactericidal effect of amoxicillin.

Adverse Effects (Side Effects):

  • Common: Hyperactivity, agitation, insomnia, dizziness; skin rashes (maculopapular rash, exfoliative dermatitis, urticaria, hypersensitivity vasculitis); diarrhea, nausea, vomiting; changes in blood counts like anemia, thrombocytopenia (low platelets), leucopenia (low white blood cells), and agranulocytosis.
  • Potentially Fatal: Neuromuscular hypersensitivity (severe allergic reaction affecting nerves and muscles); pseudomembranous colitis (a severe type of colon inflammation).
