Description

Uses:
Pacitane is used for:

  • Parkinson’s disease: to manage symptoms such as tremors, stiffness, and rigidity
  • Drug-induced extrapyramidal symptoms (EPS): such as muscle spasms, restlessness, and dystonia, usually caused by antipsychotic medications
  • May also be prescribed in combination with other Parkinson’s medications like levodopa

How It Works:
Trihexyphenidyl works by blocking acetylcholine, a neurotransmitter that is overactive in Parkinson’s disease and EPS. This helps restore the balance between dopamine and acetylcholine in the brain, improving motor control and reducing tremors and stiffness.

Dosage:

  • Starting dose is usually 1 mg daily, gradually increased to 6–10 mg/day depending on the condition and patient response
  • Taken 2–3 times per day, preferably after meals
  • Dosage should be individualized under a doctor’s supervision

Common Side Effects:

  • Dry mouth
  • Blurred vision
  • Constipation
  • Drowsiness or dizziness
  • Nausea or upset stomach

Serious Side Effects (rare):

  • Confusion, hallucinations, or memory problems (especially in elderly patients)
  • Urinary retention
  • Rapid heartbeat (tachycardia)
  • Eye pain or vision changes (possible angle-closure glaucoma)

Precautions:

  • Caution in patients with glaucoma, prostate enlargement, heart disease, or elderly
  • May cause heat intolerance due to reduced sweating—avoid overheating
  • Not advised during pregnancy or breastfeeding unless clearly needed
  • Avoid alcohol and activities requiring alertness until effects are known

Drug Interactions:

  • May enhance side effects of other anticholinergics (e.g., antihistamines, tricyclic antidepressants)
  • May reduce the effectiveness of levodopa if not properly balanced
  • Increases CNS side effects when combined with alcohol, sedatives, or opioids
