Wymox Powder For Suspension (Amoxicillin)

Indications (What Wymox is Used For):

Wymox Powder For Suspension is an antibiotic used to treat various bacterial infections, including:

• Throat infections: Pharyngitis (sore throat)
• Ear infections: Acute otitis media
• Sinus infections: Acute bacterial sinusitis
• Heart infections: Endocarditis (infection of the heart lining or valves)
• Serious infections: Anthrax
• Sexually transmitted infections (STIs): Chlamydial cervicitis and Chlamydial urethritis, Acute uncomplicated gonorrhea
• Tick-borne diseases: Lyme disease
• Dental infections: Dental abscesses
• Gastrointestinal infections: Salmonellosis (Salmonella infection), Typhoid fever
• Respiratory infections: Infections of the Lower Respiratory Tract
• Skin infections: Skin and Skin Structure infections
• Urinary and reproductive system infections: Genitourinary Tract infections
• Other common infections: Ear, Nose, and Throat infections

Dosage and Administration:

Adults:

• For most susceptible infections (taken by mouth): 250-500 mg every 8 hours.
• For uncomplicated gonorrhea (taken by mouth, with probenecid): A single dose of 3 grams.
• For dental abscesses (taken by mouth): 3 grams initially, followed by another 3 grams 8 hours later.
• For uncomplicated acute urinary tract infections (UTI) (taken by mouth): 3 grams initially, followed by another 3 grams 10-12 hours later.
• To prevent endocarditis before dental procedures (taken by mouth): A single dose of 2 or 3 grams, 1 hour before the procedure.
• For severe or recurrent respiratory tract infections (taken by mouth): 3 grams twice daily.
• For H. pylori infection (taken by mouth): 500 mg three times a day. This is usually given in combination with other medications like metronidazole or clarithromycin, and a bismuth compound or an acid-reducing drug.
• For susceptible infections (given by injection into a vein (IV) or muscle (IM)): 500 mg every 8 hours.
• For Listerial meningitis (given by injection into a vein (IV), along with other antibiotics): 2 grams every 4 hours for 10-14 days.

Children (taken by mouth):

• Standard dose: 40–45 mg per kg of body weight per day, divided into doses given every 8-12 hours.
• High dose: 80–90 mg per kg of body weight per day, with a maximum daily dose of 4 grams, divided into doses given every 12 hours.
• For penicillin-resistant S. pneumoniae otitis media (ear infection): 150 mg per kg of body weight per day, divided into doses given every 8 hours.

Dosage for Kidney Impairment (Renal Dose):

• Patients on hemodialysis: Should receive 250-500 mg every 24 hours, plus an additional dose during and after each dialysis session.
• Creatinine Clearance (CrCl) 10-30 ml/min: 250-500 mg every 12 hours.
• Creatinine Clearance (CrCl) <10 ml/min: 250-500 mg every 24 hours.

How to Take Wymox:

Wymox can be taken with or without food. Taking it with meals may help your body absorb it better and can reduce stomach upset.

Contraindications (When You Should NOT Use Wymox Powder For Suspension):

• If you have a known hypersensitivity (severe allergic reaction) to amoxicillin or any other penicillin antibiotic.

Precautions (Things to Be Aware Of):

• Use with caution if you have kidney or liver disease.
• Use with caution during pregnancy and breastfeeding. Amoxicillin can pass into breast milk, so caution is advised.
• Use with caution if you have infectious mononucleosis, as there’s a higher chance of developing a rash.

Interactions with Other Medications:

• May reduce the effectiveness of oral contraceptives (birth control pills).
• May increase the effect of anticoagulants (blood thinners), potentially increasing bleeding risk.
• Increased risk of allergic reactions when taken with allopurinol (a medication for gout).
• Increased and prolonged levels of amoxicillin in the blood when taken with probenecid (a medication that can affect how kidneys excrete drugs).
• Other antibiotics like chloramphenicol, macrolides, sulfonamides, and tetracyclines may interfere with the bacteria-killing effect of amoxicillin.

Adverse Effects (Possible Side Effects):

• Common: Hyperactivity, agitation, insomnia (difficulty sleeping), dizziness; skin rashes (e.g., maculopapular rash, exfoliative dermatitis, hives/urticaria, hypersensitivity vasculitis); diarrhea, nausea, vomiting; changes in blood counts such as anemia, thrombocytopenia (low platelets), leucopenia (low white blood cells), and agranulocytosis.
• Potentially Fatal (Serious and life-threatening): Neuromuscular hypersensitivity (a severe allergic reaction affecting muscles and nerves); pseudomembranous colitis (a severe inflammation of the colon).

Mechanism of Action (How Wymox Works):

Amoxicillin works by targeting bacteria’s cell walls. It binds to specific proteins within the bacteria (called penicillin-binding proteins or PBPs), which are essential for making the peptidoglycan layer of the cell wall. By stopping this process, amoxicillin causes the bacterial cell wall to become weak and the bacteria to break open (lysis) and die.
