  • Requires careful consideration in patients with renal and hepatic impairment.
  • Pregnancy and lactation: Use with caution. The drug crosses into breast milk; consider discontinuing the drug or using an alternative.

Interactions

  • Propantheline bromide can delay absorption and increase peak serum concentration of Ranitidine.
  • Ranitidine minimally inhibits the metabolism of coumarin anticoagulants, theophylline, diazepam, and propanolol.
  • May alter the absorption of pH-dependent drugs (e.g., ketoconazole, midazolam, glipizide).
  • May reduce bioavailability when taken with antacids.

Possible Side Effects

Common (1-10%):

  • Headache (3%)

Less Common (<1%):

  • Abdominal pain, agitation, alopecia, confusion, constipation, diarrhea, dizziness, hypersensitivity reaction, nausea, vomiting.

Rare (Frequency Not Defined, but potentially serious):

  • Anemia, necrotizing enterocolitis (in fetus or newborn), pancreatitis, thrombocytopenia, pancytopenia, agranulocytosis, acquired immune hemolytic anemia, arthralgia, myalgia.

Potentially Fatal:

  • Anaphylaxis, severe hypersensitivity reactions.

Mechanism of Action

Ranitidine works by competitively blocking histamine at the H2-receptors located on the gastric parietal cells. This action inhibits the secretion of gastric acid. It does not affect pepsin secretion, pentagastrin-stimulated intrinsic factor secretion, or serum gastrin levels.
