Synogel Syrup is an oral medication primarily used to relieve digestive discomfort. It’s a combination of Aluminum Hydroxide, Magnesium Hydroxide, and Simethicone.

Uses

Synogel Syrup is prescribed for:

  • Dyspepsia (indigestion)
  • Flatulence (gas) and abdominal bloating

Dosage

Adults (for Dyspepsia, Abdominal Bloating):

  • Take 10-20 mL by mouth every 4-6 hours.
  • It’s best taken 1 hour before or 3 hours after meals.
  • Alternatively, chew 2-4 tablets every 4-6 hours, but do not exceed 12 tablets in 24 hours.

Adults (for other conditions):

  • As an Antacid: Up to 1 gram per day.
  • For Hyperphosphatemia in chronic renal failure: Up to 10 grams per day, divided into doses taken with meals.

Administration

  • Synogel Syrup should be taken on an empty stomach.

Who Should Not Take It (Contraindications)

  • Do not use if you have a known hypersensitivity (allergy) to aluminum salts.

Important Precautions

  • Use with caution if you have chronic renal impairment, congestive heart failure (CHF), edema, cirrhosis, or are on a low-sodium diet.
  • Patients with recent gastrointestinal hemorrhage should also use with caution.
  • To minimize drug interactions, take Synogel 2-3 hours before or after any other medications.
  • Pregnancy and Lactation: If you are pregnant or breastfeeding, consult your doctor before using Synogel. It’s unknown if it passes into breast milk.

Potential Drug Interactions

Synogel can affect how other medications are absorbed.

  • Increased Absorption:

    • Aluminum Hydroxide: Its absorption can be increased by citrates or ascorbic acid.
  • Decreased Absorption (take Synogel at least 2 hours apart from these):

    • Aluminum Hydroxide may decrease the absorption of: allopurinol, tetracyclines, quinolones, cephalosporins, bisphosphonate derivatives, corticosteroids, cyclosporine, delavirdine, iron salts, imidazole antifungals, isoniazid, mycophenolate, penicillamine, phosphate supplements, phenytoin, phenothiazines, and trientine.
    • Magnesium Hydroxide may decrease the absorption of: tetracyclines and bisphosphonates.
  • Simethicone: No well-documented interactions.

Possible Side Effects

Common side effects include:

  • Aluminum Hydroxide: Chalky taste, constipation, fecal impaction, stomach cramps, nausea, vomiting, rebound hyperacidity, aluminum intoxication, hypophosphatemia, milk-alkali syndrome, and osteomalacia.
  • Magnesium Hydroxide: Diarrhea and hypermagnesemia.
  • Simethicone: Loose stools.

How It Works (Mechanism of Action)

  • Aluminum Hydroxide: Works by neutralizing stomach acid (hydrochloric acid), increasing the stomach’s pH.
  • Magnesium Hydroxide: Acts as a laxative by drawing water into the intestines, which softens stool and promotes bowel movements. It also neutralizes stomach acid.
  • Simethicone: Reduces the surface tension of gas bubbles in the digestive system, helping them to disperse and preventing gas pockets.
