Description
Spas Forte 20 Tablet, containing Hyoscine Butylbromide 20mg, is manufactured by Quest Pharmaceuticals Pvt. Ltd. It is available in Nepal, typically in packs of 100 tablets, with a retail price of 20.00 NPR per tablet. This medication is primarily used to relieve spasms and associated pain in the gastrointestinal and genitourinary tracts.
Dosage Information:
- Adult Oral Dose: For gastrointestinal and genitourinary spasms, the typical adult oral dose is 20 mg, taken 4 times daily.
- Adult Parenteral Dose (IM/IV): For gastrointestinal and genitourinary spasms, adults can receive 20 mg via intramuscular (IM) or intravenous (IV) injection. This dose may be repeated after 30 minutes if needed.
- Maximum Daily Dosage: The maximum daily dose should not exceed 100 mg.
- Child Oral Dose (6-12 years): For gastrointestinal and genitourinary spasms, children aged 6 to 12 years should take 10 mg, three times daily (tid).
Contraindications (When NOT to Use Spas Forte 20):
Do NOT use Spas Forte 20 if you have any of the following conditions:
- Narrow-angle glaucoma
- Acute haemorrhage (severe bleeding)
- Paralytic ileus (a condition where the bowel muscles don’t work properly)
- Tachycardia (rapid heart rate) due to cardiac insufficiency
- Myasthenia gravis (a chronic autoimmune neuromuscular disease)
Precautions (Use with Caution):
Inform your doctor if you have any of the following conditions, as Spas Forte 20 should be used with caution:
- Hepatic (liver) or renal (kidney) disease
- Pyloric stenosis (narrowing of the opening from the stomach to the small intestine)
- Urinary retention or prostatic hyperplasia (enlarged prostate)
- Psychosis or seizure disorders
- Ulcerative colitis
- Coronary artery disease, tachyarrhythmias, heart failure, or hypertension (high blood pressure)
- Special caution is advised for elderly individuals, children, pregnant individuals, and those who are breastfeeding.
Interactions with Other Medications:
- Additive sedative effects may occur when Spas Forte 20 is taken with alcohol or other CNS depressants.
- The effects of Spas Forte 20 may be reduced when taken concurrently with acetylcholinesterase inhibitors (such as donepezil, galantamine, rivastigmine, tacrine).
- Potentially Fatal Interaction: The effect of Hyoscine Butylbromide can be potentiated (made stronger) by other anticholinergic drugs and tricyclic antidepressants (TCAs).
Adverse Effects (Side Effects):
Side effects associated with Hyoscine Butylbromide can include:
- Cardiovascular: Flushing, postural hypotension (dizziness upon standing), tachycardia (fast heart rate), fibrillation. Rarely, psychotic reactions.
- Central Nervous System: Dizziness, drowsiness, fatigue, headache, memory loss, tremor, weakness.
- Dermatological: Dry skin, erythema (redness), increased sensitivity to light, rash.
- Gastrointestinal: Bloatedness, constipation, dry throat, dysphagia (difficulty swallowing), nausea, vomiting, xerostomia (dry mouth).
- Genitourinary: Dysuria (painful urination), urinary retention.
- Ophthalmic (Eye-related): Impaired accommodation (difficulty focusing), blurred vision, cycloplegia (paralysis of the eye’s focusing muscle), dryness, narrow-angle glaucoma, increased intraocular pain, itching, photophobia (light sensitivity), pupil dilation.
- Other: Dry nose, decreased diaphoresis (sweating), heat intolerance. Ophthalmic-specific: Somnolence, dermatitis, oedema (swelling), exudate, follicular conjunctivitis, increased IOP (intraocular pressure), local irritation, photophobia, vascular and respiratory congestion.
Potentially Fatal Side Effects: In severe cases, Hyoscine Butylbromide can lead to CNS depression, coma, and circulatory and respiratory failure.
Mechanism of Action (How it Works):
Hyoscine competitively blocks muscarinic receptors in the body. This action has both central and peripheral effects, leading to the relaxation of smooth muscle and a reduction in gastric and intestinal motility (the movement of food through the digestive tract).
