LZG 30 Capsule, with the generic name Lansoprazole, is a proton pump inhibitor (PPI) manufactured by G.D. Laboratories Pvt. Ltd. It’s available in Nepal.

Uses (Indications)

LZG 30 Capsule is used to treat conditions related to excessive stomach acid, including:

  • Peptic ulcers (duodenal and gastric ulcers)
  • H. pylori infection (as part of combination therapy)
  • Gastro-oesophageal reflux disease (GERD)
  • Hypersecretory conditions (like Zollinger-Ellison syndrome)
  • Acid-related dyspepsia (indigestion)
  • NSAID-induced ulcers and their prophylaxis
  • Erosive oesophagitis (inflammation and erosion of the esophagus)

How it Works (Mechanism of Action)

Lansoprazole, a substituted benzimidazole, works as a proton pump inhibitor (PPI). It blocks the final step of acid secretion in your stomach by inhibiting the H+/K+ ATPase enzyme system in the gastric parietal cells. This action effectively reduces both basal (resting) and stimulated acid production.

Dosage Information

Administration: Take LZG 30 Capsule on an empty stomach, before meals.

Adults:

  • Peptic Ulcer:
    • Duodenal Ulcer: 30 mg once daily in the morning for up to 4 weeks.
    • Gastric Ulcer: 30 mg once daily in the morning for up to 8 weeks.
    • Maintenance: 15 mg once daily.
  • Zollinger-Ellison Syndrome (Hypersecretory Conditions): Initially, 60 mg once daily in the morning. Your dose will be adjusted as needed. Daily doses over 120 mg should be split into two doses.
  • Acid-Related Dyspepsia: 15-30 mg once daily in the morning for 2-4 weeks.
  • Gastro-oesophageal Reflux Disease (GERD): 15-30 mg once daily in the morning for 4-8 weeks.
    • Maintenance: 15-30 mg once daily, depending on your response.
  • Prophylaxis of NSAID-Induced Ulcers: 15-30 mg once daily in the morning.
  • H. pylori Infection (1-week triple therapy): 30 mg twice daily, combined with either:
    • Clarithromycin 500 mg twice daily and Amoxicillin 1 g twice daily, OR
    • Clarithromycin 250 mg twice daily and Metronidazole 400 mg twice daily.
  • Erosive Oesophagitis: 30 mg once daily in the morning for up to 8 weeks. An additional 8 weeks may be given if not healed.
    • Maintenance: 15 mg once daily.
  • NSAID-Associated Ulceration: 30 mg once daily in the morning for 4-8 weeks.

Elderly: No dosage adjustment is typically needed.

Hepatic Impairment: If you have severe liver impairment, the maximum dose is 30 mg per day.

Children (Oral):

  • Gastro-oesophageal Reflux Disease (GERD) & Erosive Oesophagitis:
    • 1-11 years:
      • Under 30 kg: 15 mg once daily in the morning for up to 12 weeks.
      • Over 30 kg: 30 mg once daily in the morning for up to 12 weeks.
      • Your doctor may increase doses up to 30 mg twice daily if symptoms persist after 2 or more weeks of treatment.
    • 12-17 years:
      • GERD: 15 mg once daily for up to 8 weeks.
      • Erosive Oesophagitis: 30 mg once daily for up to 8 weeks.

Renal Impairment: No dosage adjustment is needed for kidney impairment.

Important Considerations

Do Not Use If You Have (Contraindications):

  • Known hypersensitivity to Lansoprazole or any of its components.

Use with Caution If You Have (Precautions):

  • Liver impairment.
  • Your doctor should rule out gastric malignancy (stomach cancer) before starting treatment.
  • Pregnancy and Lactation: Use with caution. It’s not known if Lansoprazole is distributed into breast milk; nursing is generally not recommended.

Potential Drug Interactions

  • Increased Risk of Hypomagnesaemia: If taken with diuretics (water pills) and digoxin.
  • Decreased Plasma Concentration: May reduce the effectiveness of erlotinib, dasatinib, and lapatinib.
  • Decreased Bioavailability: May reduce the absorption of itraconazole and ketoconazole.
  • Increased Plasma Concentration: May increase the levels of cilostazol and methotrexate in your blood.
  • Reduced Bioavailability: Antacids and sucralfate may reduce the absorption of LZG 30; take these at different times.
  • Potentially Fatal Interaction: May significantly decrease the serum levels and pharmacological effects of rilpivirine and atazanavir.

Possible Side Effects (Adverse Effects)

Common (1-10%):

  • Headache (3-7%)
  • Diarrhea (1-5%)
  • Constipation (1-5%)
  • Nausea (1-3%)
  • Abdominal pain (1-3%)

Less Common (<1%):

  • Anxiety, angina, palpitations, syncope (fainting), edema (swelling), anorexia (loss of appetite), dry mouth, tenesmus (straining with bowel movements), flatulence (gas), melena (dark, tarry stools), myalgia (muscle pain), tinnitus (ringing in ears), allergic reaction.
