- Lactation (breastfeeding): Gentamicin is excreted in breast milk, though no adverse effects on nursing infants have been reported. Use with caution and consult your doctor.
- Monitoring: Plasma concentrations of gentamicin should be monitored in patients receiving high doses or prolonged courses, as well as in infants, elderly patients, patients with renal impairment, cystic fibrosis, or significant obesity. Auditory (hearing) and renal (kidney) functions should also be monitored.
Interactions:
- Synergistic with: Ampicillin, benzylpenicillin, and other beta-lactam antibiotics (meaning they work better together).
- Increased risk of severe respiratory depression: When used concurrently with anesthetics or opioids.
- May reduce renal clearance of zalcitabine.
- May induce hypocalcemia (low calcium levels): When used with bisphosphonates.
- Not to be used with agalsidase alfa or beta: As it may inhibit alpha-galactosidase activity.
- Potentially Fatal Interactions:
- Increased incidence of ototoxicity: When combined with ethacrynic acid and furosemide.
- Potentiation of nephrotoxicity: Cephalosporins, ciclosporin, cisplatin, vancomycin, hydrocortisone, and indomethacin can increase the risk of kidney damage.
- Potentiates neuromuscular blocking agents: Can enhance their effects.
Possible Side Effects:
- Allergic contact dermatitis
- Erythema (redness)
- Pruritus (itching)
How Gentime Cream Works (Mechanism of Action)
Gentamicin is an aminoglycoside antibiotic. It works by binding to the 30s and 50s ribosomal subunits of susceptible bacteria. This action disrupts protein synthesis, making the bacterial cell membrane defective and ultimately killing the bacteria.
