Indications: Gastrium DT Tablet is used to treat a variety of conditions, including:

  • Nausea and Vomiting (including that associated with gastroparesis)
  • Non-Ulcer Dyspepsia
  • Gastritis
  • Migraine (for symptomatic relief of nausea and vomiting)
  • Lactation (to promote milk production, though its use for this indication should be carefully considered and discussed with a healthcare professional)

Dosage and Administration:

Gastrium DT Tablet should be taken on an empty stomach, preferably 15-30 minutes before meals.

Adults (Oral):

  • Nausea and Vomiting: 10-20 mg every 4-8 hours. Maximum dose: 80 mg/day.
  • Non-Ulcer Dyspepsia: 10 mg three to four times daily. Maximum dose: 20 mg three to four times daily. The last dose should be taken at bedtime.
  • Migraine: 20 mg every 4 hours. Maximum: 4 doses per 24 hours.

Adults (Rectal):

  • Nausea and Vomiting: 60 mg twice daily.

Elderly:

  • No dosage adjustment is typically needed.

Hepatic Impairment:

  • No dosage adjustment is typically needed.

Children (Oral):

  • 0.2 – 0.4 mg/kg every 4-8 hours daily.

Children (Rectal):

  • <12 years and >15 kg: 30 mg twice daily.
  • >12 years and >35 kg: 60 mg twice daily.

Renal Impairment:

  • Mild to Moderate: No dosage adjustment is typically needed.
  • Severe: Dosing should be reduced to 1-2 times daily with prolonged treatment.

Contraindications:

  • Hypersensitivity to domperidone or any component of the formulation.
  • Gastrointestinal hemorrhage, mechanical obstruction, or perforation.
  • Patients with prolactin-releasing pituitary tumors.
  • Chronic administration or routine prophylaxis of postoperative nausea and vomiting.

Precautions:

  • Use with caution in patients with pheochromocytoma due to the risk of hypertensive crisis.
  • Use with caution in children younger than 2 years.
  • Exercise caution in elderly patients and those with renal or hepatic impairment.
  • Intravenous administration carries a risk of cardiac arrhythmias and hypokalemia.
  • Pregnancy and Lactation: Use during pregnancy and lactation should be carefully considered and discussed with a healthcare professional.

Interactions:

  • May antagonize the hypoprolactinemic effect of bromocriptine.
  • Opioid analgesics and antimuscarinics may antagonize the prokinetic effect of domperidone.
  • Potentially Fatal: Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, ritonavir) may significantly increase serum domperidone levels, raising the risk of QT prolongation and serious cardiac arrhythmias.

Adverse Effects:

  • Common: Drowsiness, extrapyramidal reactions, galactorrhea, gynecomastia, constipation or diarrhea, lassitude, decreased libido, skin rash, itching.
  • Potentially Fatal/Serious: Convulsions, arrhythmias and cardiac arrest (especially in patients with pre-existing cardiovascular disease or hypokalemia, or those on cancer chemotherapy), seizures, hypertensive crisis (in patients with pheochromocytoma).
