E Cin Suspension is an antibiotic containing Erythromycin, manufactured by Lomus Pharmaceutical Pvt. Ltd. It comes in a 60ml bottle and is available in Nepal.
Uses (Indications)
E Cin Suspension is used to treat a variety of susceptible bacterial infections, including:
- Respiratory Tract Infections: Pneumonia, Sinusitis, Bronchitis, Pertussis (whooping cough), Legionnaire’s disease.
- Gastrointestinal Infections: Enteritis, Severe or chronic diarrhea.
- Sexually Transmitted Infections (STIs): Chlamydial infections, Chancre, Uncomplicated urethral, endocervical, or rectal infections (gonococcal and non-gonococcal), Early syphilis.
- Other Infections: Diphtheria, Trench fever, Rheumatic fever, Neonatal conjunctivitis.
- Skin Conditions: Acne.
- Prophylaxis: Bacterial endocarditis (before certain procedures).
How to Use (Administration & Dosage)
Reconstitution: To prepare the oral suspension, add 77 mL of water to the powder and shake vigorously to make 100 mL of suspension.
Administration: E Cin Suspension is best taken on an empty stomach, at least 30 minutes and preferably 2 hours before meals, to ensure optimal absorption.
Adult Dose:
- General Infections: Typically 250 mg every 6 hours, or 500 mg every 12 hours. Dosing may be increased up to 4 g/day based on infection severity.
- Upper/Lower Respiratory Tract Infections (mild to moderate): 250 to 500 mg four times a day for 10 days.
- Respiratory Tract Infections (due to Mycoplasma pneumoniae): 500 mg every 6 hours for 5 to 10 days; severe infections may require up to 3 weeks.
- Skin and Soft Tissue Infections (mild to moderate): 250 to 500 mg four times a day for 10 days.
- Acute Pelvic Inflammatory Disease (due to Neisseria gonorrhoeae): 500 mg four times a day for 10 to 14 days.
- Urogenital Infection in Pregnancy (due to Chlamydia trachomatis): 500 mg four times daily for 7 days OR 250 mg four times daily for 14 days.
- Uncomplicated Urethral, Endocervical, or Rectal Infections: 500 mg four times daily for 7 days OR 250 mg four times daily for 14 days.
- Non-gonococcal Urethritis: 500 mg four times daily for at least 7 days.
- Uncomplicated Neisseria gonorrhoeae infections (including PPNG): 500 mg four times a day for 7 days.
- Early Syphilis (primary, secondary, or latent <1 year): 500 mg four times a day for 14 days.
- Severe or Chronic Diarrhea: 500 mg four times a day for 7 days.
- Rheumatic Fever (prevention): 250 mg twice daily.
- Bacterial Endocarditis (prophylaxis): 1 g 2 hours before procedures, then 500 mg 6 hours after the initial dose.
- Acne: 500 mg twice daily for 3 months, then reduced to 250 mg twice daily for another 3 months.
Child Dose:
- Oral: 50 mg/kg/day, given every 6-8 hours.
Renal Impairment:
- No dose adjustment is necessary for renal impairment.
Hepatic Impairment:
- Use with caution.
Important Considerations
- Do Not Use If (Contraindications): Do not use E Cin Suspension if you have a known hypersensitivity to Erythromycin, porphyria, hepatic impairment, or during pregnancy.
- Precautions:
- Increased risk of cholestatic hepatitis (liver inflammation) if treatment exceeds 10 days or in patients with a history of erythromycin use.
- Use with caution if you have a history of hepatic disorders, arrhythmias, or prolonged QT interval.
- Monitor liver function during treatment.
- Avoid the estolate form of Erythromycin in liver impairment (note: this pertains to specific salt forms, ensure you are aware of the form in E Cin).
- Use lactobionate form with caution in severe renal impairment.
- May worsen muscle weakness in patients with myasthenia gravis.
- Pregnancy & Lactation:
- Pregnancy: Contraindicated.
- Lactation: Erythromycin is distributed into breast milk; use with caution. The American Academy of Pediatrics (AAP) categorizes it as compatible with breastfeeding.
- Interactions:
- May decrease plasma levels of valproic acid, potentially increasing the risk of seizures.
- Increased plasma concentrations of Erythromycin may occur with probenecid.
- Potentially Fatal: May increase prothrombin time with anticoagulants.
Possible Side Effects (Adverse Effects)
Common (1-10%):
- Abdominal pain (8%), Headache (8%), Nausea (8%), Diarrhea (7%), Rash (3%), Vomiting (3%), Dyspepsia (2%), Flatulence (2%), Pain (2%), Pruritus (1%).
Frequency Not Defined (may occur):
- Pseudomembranous colitis, Hypertrophic pyloric stenosis, Anaphylaxis, Fever, Mild allergic reactions, Urticaria, Skin eruptions, Tinnitus (ringing in ears).
Less Common (<1%):
- Cholestatic hepatitis, Confusion, Hallucinations, Hearing loss, Hypotension, Nervous system effects (including seizures), Torsade de pointes, Ventricular tachycardia, Vertigo.
Potentially Fatal:
- Hepatotoxicity, Cholestatic jaundice, Raised serum transaminases, Eosinophilia.
How It Works (Mechanism of Action)
Erythromycin inhibits bacterial protein synthesis. It does this by binding irreversibly to the 50S ribosomal subunit of susceptible bacteria, which blocks the processes needed for protein building (transpeptidation or translocation reactions). This ultimately stunts bacterial cell growth and leads to their death.
