Doverin 40 mg Tablet is an antispasmodic medication manufactured by Intas Pharmaceuticals Ltd. Its active ingredient is Drotaverine Hydrochloride. It is available in Nepal.

What is it used for?

Doverin 40 mg is prescribed to relieve pain and discomfort caused by smooth muscle spasms in various parts of the body. This includes:

• Gastrointestinal (GI) tract spasms: Such as stomach and intestinal cramps.
• Irritable Bowel Syndrome (IBS): Helps ease associated abdominal pain and discomfort.
• Dysmenorrhea: Painful menstrual periods and associated cramps.
• Biliary colics: Pain from conditions like gallstones.
• Renal colics: Pain from conditions like kidney stones.
• General abdominal pain and cramps.

How to take it:

Adults:

• Oral (Tablets): The usual dose is 1 to 2 tablets, three times daily, as directed by your doctor for spasm relief.
• Injection (for acute situations): For more rapid relief, especially in severe cases, a healthcare professional may administer 1 to 2 ampoules via intramuscular (IM) or subcutaneous (SC) injection, 1-3 times daily. For acute stone colics, it can be given as 1 or 2 ampoules by slow intravenous (IV) injection.

Children (Oral – as an antispasmodic):

• Children over 6 years: 1/2 to 1 tablet, 1-2 times daily (equivalent to 40 mg 1-2 times daily).
• Children 1-6 years: 1/4 to 1/2 tablet, 1-2 times daily (equivalent to 20 mg 1-2 times daily).

Doverin 40 mg can generally be taken with or without food. For consistent results, it may be beneficial to take it at the same time each day if prescribed regularly.

Important Precautions and Contraindications:

Do NOT use Doverin 40 mg if you have:

• Severe kidney, liver, or heart dysfunction (e.g., severe heart failure).
• Porphyria (a rare genetic disorder).
• Known hypersensitivity or allergy to Drotaverine or any other ingredient in the tablet.

Use with Caution and consult your doctor if you have:

• Any existing kidney, liver, or heart issues (even if not severe).
• Low blood pressure, as it may cause a further drop.

Drug Interactions:

• Doverin 40 mg may reduce the effectiveness of levodopa (a medication for Parkinson’s disease).

How it Works (Mechanism of Action):

Doverin 40 mg (Drotaverine) is a direct-acting antispasmodic. It works by inhibiting an enzyme called phosphodiesterase-IV (PDE4), which is specific to smooth muscles. By inhibiting PDE4, it leads to an increase in cyclic adenosine monophosphate (cAMP) levels within the smooth muscle cells. This increase in cAMP promotes the relaxation of smooth muscle tissues, thereby alleviating spasms and the pain associated with them. It also has minor effects on calcium channels, further contributing to muscle relaxation.
