  • Injection (IM/IV): Typically 50-100mg every 4-6 hours.
  • Doses might be adjusted for elderly patients, and those with liver or kidney problems.
  • You can take it with or without food.

Domadol Capsule is used to manage moderate to severe pain and postoperative pain.

 

How to Use (Dosage)

 

Important: Always follow your doctor’s specific dosage instructions.

 

Adult Dosage

 

  • Oral (by mouth) for Moderate to Severe Pain:
    • Immediate Release:
      • Chronic Pain: Start with 25mg every morning. Your doctor may increase this by 25-50mg daily every three days, up to 50-100mg every 4-6 hours as needed. Do not exceed 400mg per day.
      • Acute Pain: Take 50-100mg every 4-6 hours as needed. Do not exceed 400mg per day.
    • Extended Release: Start with 100mg once daily. Your doctor may increase this by 100mg daily every five days, up to a maximum of 300mg per day.
  • Parenteral (IM/IV Injection) for Moderate to Severe Pain:
    • Administer 50-100mg every 4-6 hours over 2-3 minutes.
  • Parenteral (IM/IV Injection) for Postoperative Pain:
    • Start with 100mg, then 50mg every 10-20 minutes if needed, up to 250mg in the first hour (including the initial dose). Afterward, take 50-100mg every 4-6 hours, with a total daily limit of 600mg.

 

Special Populations

 

  • Elderly (over 75 years): The time between doses may need to be longer.
  • Liver Impairment (Severe): For immediate release, take 50mg every 12 hours. Extended release is not advised.

 

Child Dosage

 

  • Moderate to Severe Pain (Immediate Release):
    • Under 17 years: Not enough information on safety and effectiveness.
    • Over 17 years (Acute): 50-100mg every 4-6 hours as needed. Do not exceed 400mg per day.
    • Over 17 years (Chronic): Start with 25mg every morning, then increase by 25-50mg daily every three days (in separate doses) up to 50-100mg every 4-6 hours as needed. Do not exceed 400mg per day.
  • Extended Release:

Who Should NOT Use It (Contraindications)

 

Do not use Domadol Capsule if you:

  • Have suicidal thoughts.
  • Are acutely alcoholic.
  • Have head injuries.
  • Have increased pressure in your skull.
  • Have severe kidney impairment.
  • Are breastfeeding.

 

Important Warnings and Precautions

 

Use Domadol with caution if you have:

  • Emotional disturbance or depression.
  • A history of epilepsy or are at risk of seizures.
  • A head injury or increased pressure in your skull.
  • Kidney or liver problems.
  • Are elderly.
  • Are pregnant or breastfeeding.

Patient Advice: This medicine might affect your ability to drive or operate machinery.

Monitoring: Your doctor may monitor your pain relief, breathing rate, blood pressure, pulse, and watch for signs of tolerance, abuse, or suicidal thoughts.

Lactation: This drug passes into breast milk in high amounts and is not recommended for breastfeeding mothers.

 

Drug Interactions

 

Taking Domadol with certain other medications can increase risks:

  • Increased risk of convulsions or serotonin syndrome: With SSRIs, SNRIs, TCAs, and other drugs that lower the seizure threshold (like bupropion, mirtazapine, tetrahydrocannabinol).
  • Decreased Domadol levels in blood: With carbamazepine.
  • May strengthen the antidepressant effects of norepinephrine, 5-HT agonists, or lithium.
  • Increased risk of bleeding (INR and bruising): With blood thinners like warfarin.

Potentially Fatal Interaction:

  • Increased risk of seizures and serotonin syndrome when taken with MAOIs (Monoamine Oxidase Inhibitors).

  • Abnormal ECG, Swelling (angioedema), Wheezing, Flushing, High blood pressure, Low blood pressure, Heart problems, Palpitations, Hives, Withdrawal symptoms.

Potentially Fatal Side Effect:

  • Respiratory depression (severely slowed breathing).

 

How It Works (Mechanism of Action)

 

Tramadol works in the brain by blocking the reuptake of norepinephrine and serotonin and increasing serotonin release. It also binds to mu-opiate receptors, which changes how your body perceives and responds to pain.
