  • Mometasone Furoate is a synthetic corticosteroid, specifically classified as a potent topical glucocorticoid. It is not an antibiotic, antifungal, or retinoid. Its primary action is to reduce inflammation and suppress immune responses in the skin.

Uses of Dermetasone Cream: Dermetasone Cream is prescribed for the relief of inflammatory and pruritic (itchy) manifestations of corticosteroid-responsive dermatoses. These include a range of skin conditions where inflammation and itching are prominent:

  • Eczema (Dermatitis): Various forms such as atopic dermatitis, contact dermatitis, seborrheic dermatitis, and discoid eczema, characterized by red, itchy, sometimes weeping or scaly patches.
  • Psoriasis: A chronic autoimmune condition that causes red, scaly patches, often silvery, on the skin.
  • Allergic Skin Reactions: Rashes and itching caused by allergic reactions.
  • Lichen Planus and Lichen Simplex: Inflammatory skin conditions that cause itchy, often purplish lesions or thickened, leathery skin.
  • Other inflammatory skin conditions: Where other, milder topical steroids have not been effective.

How it Works (Mechanism of Action): Mometasone Furoate works by:

  • Anti-inflammatory Action: It inhibits the release of various chemical mediators (like prostaglandins, leukotrienes, and cytokines) that are responsible for the inflammatory response in the skin. This leads to a reduction in redness, swelling, and heat.
  • Immunosuppressive Action: It suppresses the activity of certain immune cells in the skin that contribute to the inflammatory process, thereby calming the immune system’s overreaction.
  • Vasoconstrictive Action: It causes the constriction of small blood vessels in the skin, which further helps to reduce redness and swelling in the affected area.
  • Anti-pruritic Action: By reducing inflammation, it also helps to alleviate itching.

Side Effects: As a potent topical corticosteroid, Dermetasone Cream can cause localized and, in rare cases, systemic side effects, especially with prolonged use, application on large areas, or under occlusive dressings. Common local side effects include:

  • Burning or stinging sensation at the application site
  • Itching (pruritus)
  • Skin thinning (atrophy), which can manifest as fragile skin, visible blood vessels (telangiectasias), or purpura (bruising).
  • Skin discoloration (hypopigmentation or hyperpigmentation)
  • Stretch marks (striae)
  • Folliculitis (inflammation of hair follicles)
  • Acne-like eruptions
  • Perioral dermatitis (rash around the mouth)
  • Rosacea exacerbation

More serious (but less common) systemic side effects can occur due to absorption, especially in children or with extensive use:

  • Adrenal suppression (suppression of the body’s natural steroid production)
  • Cushing’s syndrome (characterized by weight gain, round face, etc.)
  • Delayed wound healing
  • Ocular effects like glaucoma or cataracts (if applied near the eyes, especially in children)
  • Increased susceptibility to secondary skin infections (bacterial, fungal, viral).

Precautions and Warnings:

  • For External Use Only: Do not ingest. Avoid contact with eyes, mouth, and other mucous membranes. If accidental contact occurs, rinse thoroughly with water.
  • Duration of Use: Generally, use should be limited to the shortest possible duration and applied only to the affected areas. Prolonged use, especially on the face, groin, armpits, or in children, should be avoided unless strictly supervised by a doctor.
  • Occlusive Dressings: Do not apply under bandages or occlusive dressings unless specifically instructed by a doctor, as this significantly increases absorption and the risk of side effects.
  • Contraindications: Do not use Dermetasone Cream on:
    • Individuals with a known allergy to Mometasone Furoate or any other component of the cream.
    • Untreated skin infections (bacterial, fungal, viral, parasitic, e.g., herpes simplex, chickenpox, shingles, scabies, tinea infections), as corticosteroids can worsen them.
    • Acne vulgaris or Rosacea.
    • Perioral dermatitis (dermatitis around the mouth).
    • Ulcerated skin or wounds.
    • Genital or perianal itching unless specifically diagnosed and prescribed.
    • Children under 2 years of age without strict medical supervision.
  • Pregnancy and Breastfeeding: Use during pregnancy or breastfeeding should only be considered if the potential benefit outweighs the potential risk to the fetus or infant. Consult your doctor.
  • Children: Children are more susceptible to systemic side effects due to a larger skin surface area to body weight ratio. Use with extreme caution and only as directed by a pediatrician for limited periods.
  • Discontinuation: Do not suddenly stop using the cream after prolonged use, as this can lead to a rebound flare-up of the original condition. Follow your doctor’s instructions for gradual discontinuation.
  • Irritation: If irritation, dryness, or sensitization develops, discontinue use and consult your doctor.
