Clarbact 250 Tablet is used to treat various infections, including:
- Respiratory tract infections (e.g., pharyngitis, tonsillitis, acute maxillary sinusitis, acute bacterial exacerbation of chronic bronchitis, pneumonia, community-acquired pneumonia, legionellosis)
- Skin and soft tissue infections
- Leprosy
- Peptic ulcer disease (associated with Helicobacter pylori)
- Lyme disease
- Acne
- Prophylaxis for endocarditis
How to Use
For Adults (Oral):
- Acute Exacerbation of Chronic Bronchitis: 250-500 mg by mouth every 12 hours for 7-14 days. Extended-release form: 1000 mg by mouth once daily for 7 days.
- Mycobacterial Infection (Prophylaxis and Treatment): 500 mg by mouth every 12 hours for 7-14 days. Use with other antimycobacterial drugs (e.g., rifampin and ethambutol).
- Peptic Ulcer Disease: 500 mg by mouth every 8-12 hours for 10-14 days, as part of a 2- or 3-drug combination regimen (e.g., with bismuth subsalicylate, amoxicillin, H2 receptor antagonist, or proton pump inhibitor).
- Pharyngitis, Tonsillitis: 250 mg by mouth every 12 hours for 10 days.
- Community-Acquired Pneumonia, Skin/Skin Structure Infection: 250 mg by mouth every 12 hours for 7-14 days. Extended-release form: 1000 mg by mouth once daily for 7 days.
- Endocarditis Prophylaxis: 500 mg by mouth 30-60 minutes before a surgical procedure.
For Adults (Intravenous):
- Respiratory tract infections; Skin and soft tissue infections; Susceptible infections: 500 mg twice daily for 2-5 days. Infuse over 60 minutes using a 0.2% solution. Switch to oral therapy as soon as possible.
For Children (Oral):
- 15 mg/kg/day, maximum 1 gram/day, every 12 hours.
Topical: Apply a thin layer to the affected area once daily.
Renal Impairment: If creatinine clearance (CrCl) is less than 30 ml/min, halve the dosage or double the dosing interval.
Administration:
- Standard release tablets and oral suspension: Can be taken with or without food.
- Extended-release (XL) and modified-release (MR) tablets: Should be taken with food. Swallow whole; do not chew or crush.
Important Precautions
- Do not use if you have a hypersensitivity to clarithromycin or other macrolides.
- Contraindicated in patients receiving terfenadine, astemizole, pimozide, cisapride, and ergot derivatives.
- Avoid use during pregnancy and in patients with a history of acute porphyria.
- Use with caution in patients with renal and hepatic impairment.
- Be aware of potential macrolide cross-resistance.
- Lactation: The drug is excreted in breast milk; use with caution.
