Description
Arbitel H is a prescription medication commonly used to manage high blood pressure (hypertension) and related cardiovascular conditions. It is manufactured by Micro Labs Ltd. and is available in Nepal under the brand name Arbitel H.
Composition
Each tablet contains:
- Telmisartan (40 mg or 80 mg): An angiotensin II receptor blocker (ARB) that helps relax blood vessels by blocking the action of a hormone that causes blood vessels to constrict.
- Hydrochlorothiazide (12.5 mg): A thiazide diuretic that increases urine output, helping to remove excess salt and water from the body, thereby lowering blood pressure.
Indications
Arbitel H is prescribed for:
- Hypertension: To lower high blood pressure.
- Cardiovascular risk reduction: To decrease the risk of strokes and heart attacks.
- Heart failure: To manage fluid retention and reduce the heart’s workload.
- Kidney protection: Particularly in diabetic nephropathy, to protect kidney function.
Mechanism of Action
Telmisartan: Blocks the AT1 receptors for angiotensin II, preventing vasoconstriction and aldosterone release, leading to blood vessel relaxation.
- Hydrochlorothiazide: Inhibits sodium and chloride reabsorption in the kidneys, promoting fluid loss and reducing blood volume.
Dosage & Administration
- Dosage: Typically, 1 tablet daily, with or without food.
- Administration: Swallow the tablet whole with water; do not chew or crush.
- Renal Impairment: No dose adjustment is necessary for renal impairment, but caution is advised.
Contraindications
Do not use Arbitel H if you:
- Are pregnant (especially in the 2nd and 3rd trimesters) or breastfeeding.
- Have severe hepatic impairment or biliary obstructive disorders.
- Are allergic to telmisartan, hydrochlorothiazide, or any component of the tablet.
- Are taking aliskiren (a direct renin inhibitor) in patients with diabetes.
Precautions
Use with caution in patients with:
- Renal artery stenosis or renal impairment.
- Electrolyte imbalances (e.g., low potassium or sodium).
- Liver disease.
- Volume depletion or dehydration.
- Elderly patients, who may be more sensitive to side effects.
Common side effects include:
- Dizziness or lightheadedness.
- Fatigue.
- Nausea or vomiting.
- Diarrhea.
- Headache.
- Upper respiratory tract infections.
